Crystal Lake Community Consolidated School District 47 recently issued the following announcement.
This fall, students, staff, and parents across Illinois will have an opportunity to participate in the seventh annual statewide Illinois 5Essentials Survey. The 5Essentials survey is designed to generate a detailed picture of the inner workings of Illinois schools from the student, staff, and parent perspective. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school.
The parent survey (available in English and Spanish) is now open and takes approximately 10 minutes to complete. One survey may be completed and submitted for each child by either parent. This is the only survey about your child’s school that will be given this year so we hope you will take this opportunity to provide us with your feedback. To access the survey, please visit http://survey.5-essentials.org/illinois/survey/parent/.”
If at least 20 percent of parents in each school complete the survey, a parent supplement report will be generated for the district. This report will help us understand parent perceptions of the conditions at each school and help guide improvement, especially as it relates to our new strategic plan. Your identity and survey responses will be kept completely confidential and will not be connected to you or your child.
Your opinion is important to us and we hope all of our families will participate. If you have any technical difficulties, questions or concerns, please contact 5Essentials Client Services at 1-866-440-1874 or 5essentials@uchicago.edu.
