HUNTLEY COMMUNITY SCHOOL DISTRICT 158: Reminder: Physical and Immunization Requirements for 2020-21

Huntley Community School District 158 issued the following announcement on July 31.

The Illinois Department of Public Health requires a number of immunizations and exams before students can attend school in the State of Illinois. This requirement stands even when the District is in a Remote Learning environment.  Parents are asked to please submit proof of the health records to their students’ Health Office at the school for the 2020-21 school year as soon as possible. If you have any questions please contact your school nurse.

Physical Exams/Immunizations: All students entering Preschool, Kindergarten, Sixth and Ninth grades are required to have a current physical examination completed on the approved Illinois Department of Public Health form.

All students must be current with all Illinois immunization requirements. Please refer to the Health requirements posted on the Health Services web page or consult with your physician. Please note the following:

  •  All students must show proof of receiving 2 doses of the Varicella (chickenpox) vaccine, with the first dose after their first birthday.
  • All students entering sixth and ninth grade must show proof of receiving one dose of the Meningococcal (meningitis) vaccine.
  • All students entering twelfth grade must show proof of receiving two doses of the Meningococcal (meningitis) vaccine unless the first dose was given after the age of 16, then only one dose is required.
  • All students entering grades six and twelve must show proof of receiving one dose of Tdap vaccine. This vaccine must be given on or after the child’s 11th birthday.
