Crystal Lake Community Consolidated School District 47 recently issued the following announcement.
Illinois state law requires children entering preschool, kindergarten, 6th grade and 9th grade to have a complete physical examination on file with the school health office before the first day of school or the child will not be allowed to start school. While this policy remains unchanged this year despite the COVID-19 pandemic, we have extended the deadline for submitting school physicals to October 15, 2020. Physical exams must have been completed within one year prior to the first day of school.
Upon completion of your child’s health exam, forms and immunization records may be scanned and emailed to the school nurse by visiting the medical records submission page of the District 47 website. Forms can also be mailed to your child’s school office. Forms may not be dropped off, mailed or emailed to the CORE Center.
Please note: Fall sports for District 47 middle schoolers have been cancelled due to the pandemic. Should sports be offered later in the school year and your child wants to participate, a sports physical or health exam is required. Sports physicals are valid for 395 days. State health and sports physical forms are available in medical provider offices or can be found on the IESA website.
