CRYSTAL LAKE COMMUNITY CONSOLIDATED SCHOOL DISTRICT 47: COVID-19 Daily Health Screening

Crystal Lake Community Consolidated School District 47 recently issued the following announcement.

Starting October 5th, parents/guardians of students returning to school buildings will be expected to log into ParentVue daily by 7:30am to complete a brief COVID-19 health screener for each child. Parents/guardians will be asked to check their child’s temperature and answer questions to ensure their child is free of COVID-like symptoms.The electronic form can be accessed via the ParentVue mobile app or on the web on 10/5. Daily Health Screener in ParentVue - English/Spanish

If your child has a temperature of 100.4 degrees or higher and/or has any of the following symptoms, your child should remain at home. Reminder: If you keep your child at home for any reason, please contact the school’s attendance line to report the absence. 

COVID-like symptoms include:

  • Cough
  • Chills
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache (not due to a chronic condition)
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose (not due to known allergies)
  • Nausea or vomiting
  • Diarrhea 
Next week (October 5-9, 2020), families will receive a phone call, text message and email at 7am as a reminder to complete this process. Families will receive a second notification if this process isn’t completed by 7:30am. 

IMPORTANT: If you have not logged into ParentVue this year or have forgotten your password, please ensure you have access. If you have forgotten your password, click the Forgot Password button and/or contact your child’s school or hd@d47.org for assistance.
