Parents of students in grades K-12, students in grades 6-12 and certified staff within Crystal Lake Elementary District 47 are invited to take a statewide survey called the 5Essentials.
The school district has partnered with the Illinois State Board of Education (ISBE) in making parents, students and staff aware of their chance to take part in the online survey, administered by UChicago Impact at the University of Chicago. The goal of the survey is to gauge school environment and learning conditions.
The online survey begins on Wednesday and runs through Jan. 16, 2017. The parent survey, available in English and Spanish, takes just 10 minutes to complete, and parents who have multiple children enrolled in District 47 are encouraged to answer a survey for each child.
The student survey will be administered during school hours sometime in December or January. Parents can opt their child out of the survey by filling out the appropriate form, which will be emailed to all parents, along with detailed information about the student survey, on Nov. 28. It will take children 25 to 30 minutes to complete the survey, and someone will be available to read the questions aloud if needed.
