Huntley Community School District 158 issued the following announcement on June 28.
An important reminder to the Parents of District 158 Students:
The Illinois Department of Public Health requires a number of immunizations and exams before students can attend school in the State of Illinois. Below is a detailed list of those requirements and the grades affected by each prerequisite. Parents are asked to please submit proof of the health records for the 2018-2019 school year as soon as possible. Summer appointments in the offices of health care providers fill up quickly, so book now.
Physical Exams/Immunizations: All students entering Preschool, Kindergarten, Sixth and Ninth grades are required to have a current physical examination completed on the approved Illinois Department of Public Health form. A current physical is one that was completed within one year of August 25, 2018. A sport physical does not meet this requirement. The parent must complete and sign the health history section on the backside of the physical form.
All students must be current with all Illinois immunization requirements. Please refer to the Health Requirements for the 2018-19 school year posted on the Health Services web page. Please note the following:
- All students entering kindergarten, grades six and twelve must show proof of receiving two doses of the Varicella (chickenpox) vaccine.
- All students entering sixth and ninth grade must show proof of receiving one dose of the Meningococcal (meningitis) vaccine.
- All students entering twelfth grade must show proof of receiving two doses of the Meningococcal (meningitis) vaccine unless the first dose was given after the age of 16, then only one dose is required.
- All students entering grades six and twelve must show proof of receiving one dose of Tdap vaccine.
Eye Examination Requirement: Illinois Eye examination reports are due by October 15th for students enrolling in kindergarten or for the first time in an Illinois public school. A current examination is within year of enrollment.
The health information must be submitted prior to attending school. The high school and middle schools must receive the information at the respective schools prior to material pick up days or materials (schedules, books, etc.) will not be released.
New students at District 158: New students must provide a current physical examination and immunization record complying with the above regulations no matter the grade in which they are enrolling. Students who are new to Illinois schools must also submit a vision exam. New students who are new to Illinois must provide their health information within 30 days of enrollment.
Religious Exemption Forms: Religious exemption forms can be found on the District 158 website. Per IL law, these forms must be filled out by the parent and signed by your health care provider.
Please follow the directions in the parent letter received in the spring for where to submit your student’s health records. If you have questions, please email your child’s school nurse.
